Information for Foraker Group Partners
We are pleased to offer The Foraker Group Benefit Plan, which went into effect on October 1, 2008. This plan is designed to promote wellness as a way to help control escalating health care costs. Employee responsibility for creating a healthy lifestyle is supported by the plan.
Here's a brief recap of what is offered:
- Three Premera Blue Cross Blue Shield medical plans: two high deductible plans with health savings account (HSA) and a traditional indemnity plan
- LifeWise Life and AD&D* (provided with medical coverage) *Accidental Death & Dismemberment
- Optional Premera dental coverage (two options)
- Optional short and long-term disability coverage through LifeWise and The Hartford, respectively
Please be aware that your organization must meet certain requirements to join the plan, including participation in a health risk management (HRM) program, minimum premium contributions, and contributions to the HSA, among others. Detailed information about the requirements and any restrictions that may apply has been forwarded to all Premera licensed insurance broker/consultants throughout the state. We also are pleased to announce that the Rasmuson Foundation is offering incentives that will make it more advantageous for Foraker Partners to join the program early. Click here to learn more about this generous gift.
We continue to conduct training sessions for both Foraker Partners and the broker/consultant community to answer questions and help you get started in the program. Dates and locations are posted on the calendar when sessions are scheduled.
