Job Description

Position Summary

The Community Resource Coordinator contributes to the vision of providing Mat-Su residents with a centralized location where they can access the resources they need to increase health and wellness within themselves and their families.  This position will be the first point of contact for Connect Mat-Su customers, whether accessed by phone or in person, and will provide social service navigation and referral to appropriate resources as identified by the customer.  The Community Resource Coordinator will be well-educated on the information and systems available through Connect Mat-Su partners and will direct and refer customers to the resource best suited to their needs.  Connect Mat-Su partners with intensive resource navigation services will be utilized for customers with unclear or multi-faceted needs.  The Community Resource Coordinator is housed in the first-floor of the Mat-Su Health Foundation building and is employed by Sultana while supervised by the Director of Connect  Mat-Su.  The Community Resource Coordinate will work closely with the Director and other Connect Mat-Su partners to develop and maintain a database of health and social services resources within the Mat-Su borough and may engage in outreach to encourage agency/provider participation in the Connect Mat-Su project.

            Responsibilities

  • Cultivate organizational and programmatic relationships to keep current with information of evolving health and social service providers in the Mat-Su.

  • Efficiently manage the overall process for referrals to Mat-Su providers

  • Communicate with and inform Connect Mat-Su visitors and callers of resources that meet their specific needs as requested, with diplomacy and sensitivity to confidentiality

  • Collaborate with the CM Network Director to identify ways to promote and make accessible the Connect Mat-Su resources throughout the entire Mat-Su Borough, including remote and online strategies.

  • Participate in the development of a robust central database of health and social services and work with Connect Mat-Su Director and other partners to continuously update.

  • Follow-through in a timely manner with community requests to create “warm hand offs” to providers, partners, appropriate entities, and investigate other opportunities for residents as appropriate.

  • Assist Connect Mat-Su Director in developing and hosting community outreach and provider collaboration events

  • Be cross-trained to be able to provide all information and referral assistance at the front desk and on the phone as needed.

  • Continually work to be trauma-informed and culturally competent, including providing translation services as needed

  • Ability to navigate high stress situations with customers, help identify primary needs and work in a collaborative format to identify resources to meet those needs

  • Ability to learn quickly, stay flexible and highly organized in fast-paced environment with competing demands while maintaining integrity of work product for self and staff

  • Ability to advance mission of the organization through detailed and effective organization skills and logistics coordination

  • Solid sense of self with demonstrated qualities of integrity, loyalty, discretion and self-motivation

  • Strong analytic skills and the ability to think strategically, politically and programmatically

  • Flexibility in responsiveness to the needs of the Connect Mat-Su director and other Connect Mat-Su partners

  • Knowledgeable of, or willingness to become, trauma-informed and culturally competent

     

    Position Qualifications

     

  • Bachelor’s degree in psychology, social work, or related field is preferred

  • A minimum of 3 years work experience, social service experience preferred, including 1 or more years of proven community outreach and coordination experience

  • Super user of Microsoft office suite, Microsoft Access and other database management systems

  • Super user of standard office communication systems, hardware, copiers, and other office equipment

  • Demonstrated skill and experience in information retrieval, information/data management, and procurement

  • Demonstrated high level interpersonal and oral/written communication skills

  • Existing relationships with, or ability to build relationships with, a cross-sectoral range of stakeholders in the local area, including senior executives
