Care Coordinator I
Anchorage, AK

$21.00 per hour+ DOE 

Our client: Committed to improving the lives of people living with Alzheimer’s Disease and related dementias,  providing valuable services and support to those living with Alzheimer’s, ensuring quality of life.  They are seeking an entry-level Care Coordinator I to assist the team with caseloads while being trained and mentored.

Would you like to: 

  • Process requests for services, provide referrals, assessments, consultation, monitoring and follow-up.
  • Develop a support plan with and for individuals enrolled in the Care Coordination service and update the plan as needed.
  • Provide case management services, working with each individual enrolled in the Care Coordination Senior In-Home grant program.  Serving low-income seniors who are considered frail or those who are experiencing Dementia at any age.  
  • Implement the support plan, coordinating with the necessary providers ensuring that each provider receives an approved plan of care.
  • Monitor client situations to ensure the individual and family are receiving the services outlined in the support plan.
  • Identify and advocate for new, improved and/or enhanced services to meet the needs of family coping with Alzheimer’s Disease and Related Dementia.
  • Assist with new referral process by accepting phone calls or appointments, collection of information, and referral to appropriate department (commonly known as “intake”).
  • Promote the Agency’s mission, goals and programs.
  • Understands and complies with all agency policies and procedures.
  • Ensure confidentiality as prescribed by agency policy and federal/state law.
  • Maintain and promote a positive professional working relationship with all employees.
  • Attend and participate in meetings, committees and training as assigned.
  • Follow timelines and documentation practices established by the Care Coordination Director.
  • Perform other duties as assigned

 Ideally you have: 

  • Bachelors, Associates degree or at least two years of study from an accredited college or university in social work, psychology, rehabilitation, nursing, health, human services or closely related field and minimum of one year of full or equivalent part time work experience with human services recipients.  The education requirement may be substituted for additional work experience (e.g. three years paid work experience with human services recipients).  Certification as a rural community health aide or practitioner and one year of full or equivalent part time work experience with human services recipients.  
  • Must become certified by the State of Alaska to provide Medicaid Waiver care coordination within six months of employment.
  • Ability to pass a state and federal criminal background check.
  • Valid Alaska driver’s license with a good driving record and proof of auto insurance.
  • Must possess computer skills with Word, Excel, Outlook, TEAMs, and databases.  
  • Knowledge of providing Alaska Medicaid services preferred.  
  • The ability to perform lifting and transferring of at least 50 pounds and demonstrate good body mechanics while performing these tasks
  • Ability to climb stairs and access individuals in non-accessible rooms or apartments

Knowledge, skills and abilities: 

  • Knowledge of or ability to learn about Senior Health, Alzheimer's Disease and related disorders and/or Intellectual Developmental Disability and/or complex medical conditions
  • Knowledge of health care or social service systems
  • Sensitivity to clients of all cultures
  • Ability to evaluate client needs and complete assessments
  • Ability to network with representatives of other agencies
  • Excellent verbal and written communication skills
  • Able to work evenings and weekends as needed
  • Completion of Alaska Training Cooperative’s Basic Concept of Care Coordination class preferred
