Job Description

General Functions:

The Case Manager serves as an integral member of the multidisciplinary treatment team, supporting participants throughout their recovery journey by delivering comprehensive case management services across Recovery Services and Behavioral Health's Continuum of Care, including both outpatient and residential programs. This position links participants to appropriate services, facilitates skill-building in community and office settings, coordinates care, and provides psychoeducation guided by each participant's individualized treatment plan. Working in partnership with our people, the Case Manager helps develop opportunities that fulfill our endless potential by using a strengths-based, culturally responsive, and trauma-informed approach to empower participants toward treatment goals and long-term success. The role requires active client engagement, meaningful rapport-building, and skill in crisis intervention, life skills training, and group facilitation.

 

Duties and Responsibilities:

Case Manager I

• Manage caseload and schedule efficiently to ensure at least 75% of total work hours are dedicated to direct-service activities, with a minimum of 50% delivering billable services and up to 25% allocated to non-billable participant support such as external care coordination, transportation, outreach, engagement, and case consultations.
• Create and maintain clinical documentation for program participants, including assessments, diagnoses, treatment plans, case staffing, discharge summaries, and progress notes, ensuring timely and accurate completion within the electronic health record system.
• Coordinate care with referral sources, family members, community agencies, and other treatment providers, maintaining collaborative relationships and following up on referrals as directed by clinical leadership.
• Assist in the coordination of participant placement and transfer into treatment levels beyond assigned programs.
• Communicate updates on case management goals, objectives, and participant progress with other treatment team members.
• Deliver individual and group case management, skills training, and psychoeducation sessions based on participant needs and treatment plans.
• Participate in and observe individual and group sessions to support participant treatment plans while supporting administration of Urine Analysis (UA) and Breath Alcohol Concentration (BrAC) testing.
• Coordinate and provide transportation of participants to activities, case management services, and program-related outings.
• Foster and maintain respectful, professional relationships with participants, alumni, family members, and peers, and represent the program at community meetings and events as needed.
• Support participants in self-administration of prescribed medication as directed by medical providers and coordinate with medical providers regarding participant care as appropriate for residential programs.
• Attend required training, support meetings, and staff development while collaborating with team members in regular staff meetings and case-staffing.
• Maintain all credentials in current and up-to-date status while demonstrating a professional demeanor at all times.
• Maintain confidentiality according to all state and federal regulations, policies, and procedures.
• Perform all related duties as needed and assigned to support program goals and operations.

 

Case Manager II

• Perform all duties and responsibilities above with an advanced level of proficiency.
• Provide support to Case Manager I staff by offering guidance, answering questions, and making recommendations regarding participant care.
• Demonstrate working knowledge of applicable state and federal regulations governing service delivery and community resources available to support participant needs.

• Working knowledge of Motivational Interviewing and trauma-informed care with experience facilitating substance use treatment groups and individual sessions.
• General knowledge and understanding of the social, health, educational, training, and cultural needs of the Alaska Native and American Indian community.

Minimum Qualifications:

• Continued employment is contingent upon completion of satisfactory state and federal fingerprint criminal background check.
• Obtain Qualified Addiction Professional (QAP) approval within three (3) years of the Provisional Provider Application approval date with the State of Alaska.
• Ability to obtain a National Provider Identifier (NPI) number and Medicaid Identification number.
• CPR certified or ability to be certified within 30 days of hire for residential positions.
• Valid Alaska driver's license and insurable under CITC's automotive insurance, which requires a driver to be at least 21 years of age and have had a driver's license for at least three years.

 

Case Manager I

• Associate's degree in Psychology, Sociology, Human Services, Social Work, or related social services field. Relevant work experience may substitute for education requirement on a year-for-year basis.
• Alaska Chemical Dependency Counselor Certification I (CDC I) through the Alaska Commission of Behavioral Health Certification at time of hire, or demonstrated ability to obtain within 24 months of hire date.

 

Case Manager II

• Associate's degree in Psychology, Sociology, Human Services, Social Work, or related social services field. Relevant work experience may substitute for education requirement on a year-for-year basis.
• Two (2) years of experience providing case management services.
• Working knowledge of ASAM and DSM-5 diagnostic tools.

 

Preferred Qualifications:

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or Substance Abuse/Addiction Counseling.
• Relevant work experience in both residential and outpatient substance use and/or behavioral health treatment settings.

• Must be able to lift up to 50 pounds occasionally, by utilizing proper lifting techniques and working in a safe manner.
